When an ER admission/discharge occurs for one of Betances’ patients an alert is sent, prompting Betances care managers to review the
patient’s discharge summary. Depending on the severity of the condition, the patient is called to schedule an office visit immediately or within 7 to 14 days. The ability to know that a patient has been admitted to an ER, allows Betances staff to intervene and better manage a patient’s condition.
